Join the Masters Team

    I as player/parent/guardian at this moment:
    Confirm that I am not a member of any other Hockey Club or have a release letter from my previous Hockey Club allowing me to join PHSOB Hockey Club. I acknowledge that by signing this membership form, I am liable for the annual membership fees due to the Hockey Section I am joining as well as the Main Club. I further acknowledge that should I intend to play for another club I shall notify the chairman in writing via the email address (chair@phsobhockeyclub.co.za) within 7 days of completing this form, failing which no refund or reduction in the annual fees will be due to me.

    I further acknowledge that the membership fees are payable in advance at the beginning of the season and while a payment plan may be in existence, it does not negate my duty to pay for the full season, no matter how many practices or games i attend for whatever reason including but not limited to injury etc.

    Acknowledge that I have familiarised myself with the code of conduct and the nature and extent of the Sport and the risks and dangers to which I (my child) may be exposed as a result of (his/her) participation in the Sport;

    Voluntarily assume the risks and dangers to which he/she may be exposed as a result of my (his/her) participation in the Sport;

    Indemnify and keep indemnified that PHSOB Hockey Club, Pretoria Boys High School and/or its employees and/or agents against all injury, loss, damage, costs, and/or expenses which I and/or any other person may sustain or incur as a result of my (my child's) participation in the Sport; and

    Agree not to hold PHSOB Hockey Club, Pretoria Boys High School and/or its employees and/or agents liable for any injury, loss, damage, costs, and/or expenses, that I (my child) may sustain or incur as a result of my participation in the Sport;

    I, as player, parent or guardian of the registered player agree to the terms and conditions above.
